OKU Registry (Children with Special Needs Registry)

OKU registry will come with an OKU Card (Orang Kurang Upaya) issued by the State Director of the Social Welfare Department. With the identity of OKU, there are a lot of benefits provided by the relevant parties.

Process of Registration:

Step 1 Get Registration forms from Welfare Department or Paediatric Department or CBR Jawa

Step 2 Fill the registration form (total 4 pieces) by Doctors from Polyclinics, Health Clinics or Hospitals.

Step 3 Prepare 3 photos and photocopied birth certificate (together with the true copy)

The forms are sent to the nearest welfare department, education department (pre-school and school age) by the parent themselves, one for clinic and one for patient record (baby card).

The OKU Card will be ready for collection within 2 weeks to 2 months depends on the locality of the initial submitting office.
